FORMS

Forms are for client safety and to provide you with the best possible service. Clients only have to fill out a "New Client" form the first time they book any facial treatment and at Nourishly Esthetics. Returning clients can update their health history using an "Existing Client" form. 

 

All forms should be completed at least 24 hours prior to their appointment.
